Health Improvement Programs

A member diagnosed with diabetes may be referred by their doctor to Diabetes Education Classes, a Certified Diabetes Educator, and / or a Registered Dietitian.  These classes and health care professionals provide an overview of diabetes, monitoring blood sugars, taking medications, meal planning and exercise to help members minimize or avoid diabetes-related complications.  The information and coaching provided through the Diabetes Management Program will help members gain the information they need to make important lifestyle changes.

Asthma and COPD affects at least five percent of our population.   A member diagnosed with these conditions may be enrolled in this program and managed by a participating physician, if they have mild symptoms or be followed by a Pulmonologist for more severe progression of the chronic condition. Management of these chronic conditions includes medication monitoring and education, lung function measurement and ongoing coaching regarding risk factors.
