Becoming a Member

Enrolling in a Medicare Advantage plan can be confusing. We want to make the process easy for you and answer questions you may have after you have enrolled. The video below will walk you through the enrollment process, what happens after you enroll and what comes next once you are a member.

​Please call 713-442-JOIN (5646). Just tell the representative you would like to enroll by phone. After a few brief questions, your call will be transferred to our phone enrollment department for completion of your enrollment.

Call 713-442-JOIN (5646) to request your application. Please be sure to complete the highlighted areas, send the white copy of the application in the envelope provided and keep the yellow copy for yourself.

Next Steps

Upon receipt of your enrollment application, we will deliver your enrollment form to our enrollment department, where all information will be verified and sent to the Centers for Medicare and Medicaid Services. Medicare must review all enrollments. When Medicare has finished its review, we will send you a letter to confirm your enrollment with KelseyCare Advantage and your effective date.
 
Within a few weeks of Medicare’s confirmation of your enrollment, we will also send you a member ID card to present at all of your doctor’s appointments. This ID card will provide important information such as copayments, coverages and plan contact information.
 
At any time during the enrollment process, if you should have a question, don’t hesitate to call one of our knowledgeable Health Plan Specialists at 713-442-JOIN (5646). 
 
We’re available:
From October 1 through February 14th, seven days a week from 8:00 a.m. to 8:00 p.m.
From February 15th through September 30th, Monday through Friday from 8:00 a.m. to 8:00 p.m.
