Providers are organized by service type and listed alphabetically by company, location, or provider name to make your search easier.

Please note that some services may require a referral or prior authorization. Emergency care never requires prior authorization. While the directory includes contracted urgent care centers and freestanding emergency rooms, members may obtain emergency or urgently needed care from any appropriate facility when needed.
