Medicare Information for Caregivers

It’s common for a family member, friend, or caregiver to be appointed as a healthcare representative for an aging loved one. This can mean managing their Medicare coverage. At KelseyCare Advantage, we make doing so as easy as possible, taking the stress out of making sure your loved one has the healthcare coverage they need.

Appointing a Healthcare Representative

Your loved one can name you as their healthcare representative by signing a form giving you legal permission to act on their behalf for organizational determinations, appeals and grievances. This form should be mailed to:

If Your Loved One Needs Improved Coverage

If your loved one is already on Medicare but would like to improve their coverage, please visit Choose Your Plan to navigate KelseyCare Advantage Medicare-approved HMO plans that give your loved one access to Kelsey-Seybold Clinic’s premier multispecialty physician group and other contracted providers.

HIPAA Release of Information Form

This form confirms your loved ones permissions that KelseyCare Advantage may disclose and discuss Protected Health Information (PHI) with you.
